3) Wij zullen u dan onze deskundige mening geven over het zorgplan dat het beste bij u past, gebaseerd op het gesprek
4) Op deze manier bent u op de hoogte van de richting waarin de zorg gaat
5) De fysieke intake wordt na de telefonische intake geboekt
